NUTRITIONAL ASSESSMENT OF OLDER HOSPITALISED PATIENTS

Nutritional assessment of older hospitalised patients

Background: Disease related malnutrition is considered a major risk factor for complications and delayed recovery in hospitalised elderly patients. Due to many studies, malnutrition is easier to prevent than to treat

Aim: To compose a model for the professional team about how to assess and treat nutritional problems related to malnutrition.

Method: A multiprofessional team started in 2001 at the Geriatric department. Members of the team: a doctor, a dietician, two nurses, two assistant nurses, an occupational therapist, a speech therapist, a manager of the hospital food section and a social worker. The commission of the team is to create visions, strategies, checklists, PM and indicators of quality applicable to the nutritional process. An executive team with members from every ward at the geriatric clinic, implements the guidelines from the professional nutritional team. The executive nutritional team has the responsibility of documentation of quality indicators such as BMI, SF-MNA, and food intake.

Results: A checklist for screening of risk of malnutrition was implemented. In the checklist it was defined what should be included in the nutritional screening, how to do it, when, by whom and where it should be documented. At admission BMI and SF-MNA were measured in 99% of the patients. Patients at risk were identified and early treatment could be initiated. In 2004 39% of the patients were identified as malnourished due to the checklist
