IMPROVEMENT OF FUNCTIONAL RE-EDUCATION PROGRAMS FOR PATIENTS WITH PARKINSON'S DISEASE

Authors evaluated risk/efficiency ratio regarding medical rehabilitation for patients with Parkinson's disease. A total of 234 patients have been selected, 52% men and 48% women, average age 62.5 years (range 45 to 80 years). Kinetic-therapy program was similar both for study and control group, but adapted and individualized according to progress capacity. Benefits have been demonstrated both on short (1 month) and long term (6 months). Exercise benefited best if it was started early and if it was continued home. There was a relative discrepancy between the obvious general mobility improvement and modest improvement in ADL. Aerobic program had to be used for a longer period since the fitness level could not be reached in less than 3 months. Several parameters (sight, hearing, economical status) could not be influenced. That was the reason perception disorders could not be properly analyzed. Strenuous exercises for patients with a certain level of fitness can induce opposite effects with reduction in confidence and premature withdrawal from program. Several parameters related to patient's family (continuity, accessibility, comprehension) have an important place in re-education of patients with Parkinson's disease. Rehabilitation program has been more efficient if it was introduced early in the course of disease because it maintained aerobic capacity and prevented stiffness and vicious postures. In conclusion, benefits have clearly outweighed risks for a functional reeducation program in patients with Parkinson's disease.
